From 24/09/20 we are legally required to use the NHS test and trace system. You must install the following app and check into the app each time you are on our facility. Please install here:

If your phone is too old to use the app, or you do not have your phone with you, the following form can be used as a back up. Anyone can fill this in on your behalf.
